What is biotin / minoxidil?

(B7 vitamin + vascular growth stimulant)

Biotin is a B vitamin required for hair, skin and nail keratin synthesis. Minoxidil stimulates hair-follicle growth and blood flow in the scalp. Together, biotin ensures the new hair that minoxidil helps grow is strong and resilient. This combination is chosen for people whose hair is thin or fragile in addition to thinning.

Synergistic

biotin strengthens new hair; minoxidil stimulates its growth

Biotin is well tolerated; minoxidil can cause body hair growth or fluid retention. Your physician monitors and adjusts minoxidil dose based on blood pressure.

How it works,
week by week.

  1. Week 1

Start at low dose (1-2.5 mg minoxidil). Your physician monitors blood pressure and overall tolerance.

  2. Month 1-2

Initial shedding may occur (hair cycle reset). Body hair may begin to increase slightly. This is normal.

  3. Month 2-4

Hair loss slows. New, fuller hair begins to grow. Body hair increase stabilizes.

  4. Month 4-6+

Results become visible. Your physician adjusts dose or adds finasteride if needed. Maintenance is long-term.

What you may notice,
and when.

  • Hair regrowth

new, fuller hairs visible by month 3-4

  • Body hair increase

common and visible; usually reverses after stopping

  • Patience required

    results peak at 6-12 months

  • Blood pressure checks

    essential; minoxidil can lower BP

Individual results vary. Benefits reflect what Zepym Health members commonly report to their physician at check-ins; they are not the results of a clinical trial.

Important safety information and common side effects

Oral minoxidil and biotin are generally well tolerated. Common effects include increased body hair (hypertrichosis), especially on forearms and face; mild fluid retention; and occasional palpitations or chest tightness. Minoxidil can lower blood pressure, so it is not used in people with uncontrolled low BP or cardiac arrhythmias. Tell your physician about heart medications, blood pressure history, and any swelling or weight gain. Stop and seek care for chest pain, severe palpitations, fainting, or signs of allergic reaction.

The research behind
oral minoxidil.

Oral minoxidil is a blood pressure medicine used off-label at low doses for hair loss. The evidence is small trials, retrospective series and pooled single-arm data, not large placebo-controlled studies.

  • Review17 studies, 634 patients

A review of oral minoxidil as the main hair-loss treatment, mostly at low doses. Androgenetic alopecia was the most studied condition. The authors concluded it was effective and well tolerated for healthy patients who struggle with topical products.

Randolph and Tosti, J Am Acad Dermatol 2021;84(3):737-46
  • Safety1,404 patients

Retrospective multicenter study of patients on low-dose oral minoxidil for at least 3 months; 67.2% women, mean age 43. Hypertrichosis was the most common effect (15.1%). Systemic effects: lightheadedness 1.7%, fluid retention 1.3%, tachycardia 0.9%. 1.2% stopped for systemic effects. No life-threatening events. No control group.

Vañó-Galván et al., J Am Acad Dermatol 2021;84(6):1644-51
  • Biotin18 case reports

Review of biotin for hair and nails. In all 18 reported cases, the patient had an underlying biotin deficiency or disease, and all improved. The authors say evidence in healthy people is lacking.

What the studies found

  • Effective and well tolerated in healthy people who struggle with topical minoxidil, in 17 studies with 634 patients (Randolph and Tosti, J Am Acad Dermatol 2021).
  • Pooled across 27 studies and 2,933 patients: 35% had significant symptom improvement and 47% some improvement; no control group (Liu et al., Front Pharmacol 2025).
  • In a 2024 randomized trial, oral 5 mg daily was superior to topical 5% on vertex photographs (24%, 95% CI 0 to 48, P=.04), but not on other measures (Oral vs topical minoxidil, JAMA Dermatol 2024).

Trial and series results are for minoxidil at low doses taken by mouth. The catalog products are the pharmacy's versions of the same drug; the biotin combination has no trial of its own, and compounded versions were not studied. Oral minoxidil is off-label for hair loss.

What this does not say

  • Side effects include hypertrichosis, fluid retention, fast heart rate and lightheadedness; the reviews also list rare serious skin reactions.
  • The biotin in the combination capsule is not shown to add hair benefit in people without a deficiency.
  • Most data are small or open series; the 2024 randomized trial below is the only head-to-head with topical minoxidil we found.

The research
behind it.

Biotin supplementation improves hair strength and reduces breakage in people with biotin deficiency; minoxidil is proven to extend the growth phase and increase hair density.

The combination targets both quantity (minoxidil) and quality (biotin) of hair. Your physician monitors for minoxidil side effects and ensures adequate biotin levels.

Frequently asked questions

Contact support
How do I take Biotin / Minoxidil Capsules?

Taken by mouth, once a day. The starting prescription is 1 capsule (5 mg / 1.25 mg) each time. Your physician sets your own dose and it is printed on your label.

What strengths does Biotin / Minoxidil Capsules come in?

The pharmacy carries 2: 5 mg / 1.25 mg, 5 mg / 3 mg. Your physician chooses which one you get.

What exactly is in it?

Biotin, Minoxidil.

How much arrives in each shipment?

30 capsules at the starting strength, shipped to your door from a licensed US pharmacy.

Why add biotin if minoxidil alone regrows hair?

Biotin ensures new hair is strong and resistant to breakage. Some people with thin, fragile hair benefit more from the combination. Your physician decides if biotin adds value for your hair type.

Why does oral minoxidil cause body hair growth?

Minoxidil works everywhere: scalp, face, arms, and elsewhere. Body hair increase is common (80% of users to some degree) but usually mild. It often subsides when you stop, though some people notice hair changes persist longer.

Can I combine oral and topical minoxidil?

Not recommended. Combining them doubles your dose and increases side effects without much added benefit. Your physician chooses one form based on your preference and response.

What if I feel dizzy or have chest tightness?

Contact your physician immediately. These are signs of low blood pressure or cardiac stress. Dose may need to be reduced or stopped. Never ignore these symptoms.
