What is coenzyme Q10?

(ubiquinone or ubiquinol)

Coenzyme Q10 is an enzyme cofactor made by your body that sits in the inner mitochondrial membrane and carries electrons in the energy-producing chain. Your cells use it to make ATP, the currency of cellular energy. Q10 levels decline with age and with statin therapy (statins inhibit Q10 synthesis). Oral Q10 supplementation increases serum levels and may restore cellular energy.

Age-related

Q10 levels drop 30-40% by age 60

Statins speed this decline. Statin users often benefit from Q10 supplementation; your physician may recommend 100-300 mg daily.

How it works,
week by week.

  1. Day 1

Your physician reviews your diet, lifestyle, lab values (including vitamin D level), and goals. Discusses which vitamins fill gaps and how to time them with meals.

  2. Week 1

Start your daily vitamins with breakfast or as directed. Take fat-soluble vitamins (D, K, CoQ10, fish oil) with food to improve absorption.

  3. Weeks 2-4

Most people tolerate vitamins well. Some notice subtle energy shifts or better joint comfort. Keep a symptom log.

  4. Month 2-3

Your physician may check follow-up labs (vitamin D level, if supplementing D) to confirm optimal dosing.

  5. Beyond

Vitamins work best on a consistent daily schedule. Annual labs and check-ins ensure your regimen matches your current needs.

What you may notice,
and when.

  • Better energy

    especially if you were deficient

  • Stronger bones

D and K work synergistically over months

  • Reduced inflammation

fish oil and Q10 support recovery

  • Improved endurance

Q10 and fish oil support exercise capacity

Individual results vary. Benefits reflect what Zepym Health members commonly report to their physician at check-ins; they are not the results of a clinical trial.

Safety first:
what to know before you start.

Important safety information and common side effects

Vitamins D, K, CoQ10, and fish oil are generally well tolerated at recommended doses. D3 at very high doses (over 10,000 IU daily long-term) can raise serum calcium; your physician sets your dose. K2 is safe; no upper limit is established. Q10 is safe; high doses may cause mild GI upset or insomnia. Fish oil can thin blood slightly at high doses; tell your physician if you take anticoagulants or antiplatelet drugs. Fish oil may cause burping or GI upset in some people. Stop and seek care for signs of hypercalcemia (nausea, vomiting, confusion) or unusual bleeding.

The research behind
coenzyme Q10.

Coenzyme Q10 is a molecule the body makes and uses in cell energy production. Its outcome evidence comes from one large trial in heart failure; for healthy adults the evidence is not outcome-based.

  • Heart failureFewer major events

Q-SYMBIO randomized 420 people with moderate to severe heart failure to 100 mg of CoQ10 three times a day or placebo, on top of standard care, for 2 years. Major cardiovascular events were 15% vs 26% (hazard ratio 0.50, 95% CI 0.32 to 0.80).

Mortensen et al., JACC Heart Fail 2014 (Q-SYMBIO)
  • Early monthsNo difference at 16 weeks

In the same trial there was no significant difference between CoQ10 and placebo at 16 weeks; the separation appeared over the 2-year follow-up.

Mortensen et al., JACC Heart Fail 2014 (Q-SYMBIO)
  • Single trialOne trial, composite endpoint

The result rests on this one trial, and its primary endpoint was a composite of cardiovascular events, not all-cause death. Death rates were also lower with CoQ10, but those were secondary outcomes.

Mortensen et al., JACC Heart Fail 2014 (Q-SYMBIO)

What the studies found

  • Fewer major heart events over 2 years in people with heart failure (Q-SYMBIO, JACC Heart Fail 2014)
  • Fewer deaths and fewer heart-failure hospital stays in the same trial (Q-SYMBIO, JACC Heart Fail 2014)
  • Reduced muscle pain, weakness, cramps and tiredness in statin users, in a pooled meta-analysis of 12 trials (Qu et al., JAHA 2018)

Q-SYMBIO tested 300 mg a day in heart failure, not in healthy adults. The capsules sold here are not the trial product in every respect, and they have not been tested for healthy people.

What this does not say

  • Only heart failure was tested; healthy adults are not covered by this trial.
  • The primary endpoint was a composite, not death alone.
  • A single trial is not a settled answer.

The research
behind it.

Q10 is essential for ATP synthesis; low levels correlate with age-related fatigue and reduced exercise tolerance. Supplementation can help restore energy in deficient individuals.

Some studies show Q10 benefits heart muscle function in people with heart disease or heart failure. The ubiquinol form (reduced Q10) absorbs better than ubiquinone.

Frequently asked questions

Contact support
How do I take CoQ10 Capsules?

Taken by mouth, once a day. The starting prescription is 1 capsule (100 mg) each time. Your physician sets your own dose and it is printed on your label.

How much arrives in each shipment?

30 capsules at the starting strength, shipped to your door from a licensed US pharmacy.

Should I take Q10 if I am on a statin?

Many physicians recommend it. Statins reduce Q10 synthesis in your body, which may contribute to statin-related muscle pain and fatigue. Q10 supplementation (100-300 mg daily) may restore energy and reduce muscle symptoms. Discuss with your physician.

Can I take all these vitamins together?

Yes. D, K, CoQ10, and fish oil are safe to take together. Take them all with food to improve absorption. Separate any other medications by 2-4 hours from fish oil if you are on blood thinners.

How do I know if I am deficient?

Vitamin D deficiency is easy to test; your physician can check your serum 25-hydroxyvitamin D level. K, Q10, and omega-3 deficiencies are harder to test precisely, but dietary assessment and symptoms guide supplementation.

Are vitamins from food better than supplements?

Ideally, you get vitamins from food. But modern diets often fall short. Supplements fill gaps. A varied diet plus targeted supplements usually works best.

How long until I notice a change?

Energy shifts may appear in weeks; bone and joint benefits take months to become obvious. Be patient and consistent; improvements build gradually.
