What is ezetimibe?

(zetia, non-statin cholesterol lowering)

Ezetimibe blocks NPC1L1, a cholesterol transporter in your intestinal wall. This prevents dietary cholesterol (which you ingest and which your body makes and recycles) from entering the bloodstream. Less intestinal cholesterol absorption means lower LDL. It does not inhibit cholesterol synthesis the way statins do; it stops it from being absorbed. This makes it an ideal partner for statin therapy or for people who cannot tolerate statins.

LDL reduction with ezetimibe monotherapy

Combined with a statin, the LDL reduction is additive, often reaching 40-50%. Ezetimibe is FDA-approved for hypercholesterolemia and is covered by most insurance.

How it works,
week by week.

  1. Day 1

Your physician reviews your LDL level, cardiovascular risk, diet, and current medications. Discusses how ezetimibe fits into your cholesterol plan.

  2. Week 1

Take your ezetimibe tablet once daily, with or without food. If combined with a statin, take both at the same time.

  3. Weeks 4-6

Ezetimibe begins lowering LDL as it blocks intestinal cholesterol absorption. No symptoms to notice; the effect is measured by lab.

  4. Week 6-8

First lipid panel: LDL, HDL, triglycerides, and total cholesterol. Your physician may adjust ezetimibe dose or add or adjust the statin based on results.

  5. Beyond

Ezetimibe works best on a stable dose. Annual lipid panels and cardiovascular check-ins ensure your plan remains optimal.

What you may notice,
and when.

  • Stable HDL and triglycerides

  • Reduced cardiovascular risk

Important safety information and common side effects

Ezetimibe is very well tolerated. Side effects are rare; the most common are mild headache and GI upset. Do not use ezetimibe if you have active liver disease or if you are taking a statin and have elevated liver enzymes. Tell your physician about all medications and any history of muscle pain or weakness (myopathy risk is higher when ezetimibe is combined with a statin). Stop and seek care for severe muscle pain, dark urine, or signs of liver damage (jaundice, abdominal pain, nausea).

The research behind
ezetimibe.

Ezetimibe blocks cholesterol absorption in the gut. Its outcome evidence comes from one large trial, IMPROVE-IT, in people after a recent acute coronary syndrome, where it was added to a statin.

What the studies found

  • Lower LDL cholesterol than statin alone, after an acute coronary syndrome (IMPROVE-IT, NEJM 2015)
  • A modest reduction in heart events over about 7 years, on top of a statin (IMPROVE-IT, NEJM 2015)
  • Recommended by the 2019 ESC/EAS guideline as an add-on when LDL stays above target (Eur Heart J 2020)
Read the full research

IMPROVE-IT tested ezetimibe 10 mg added to simvastatin, after an acute coronary syndrome. The trial product was the combination tablet, and the outcome benefit is for that setting. Ezetimibe alone has no outcome trial of its own.

What this does not say

  • The benefit was shown after an acute coronary syndrome, not in people without heart disease.
  • The absolute benefit is modest over seven years.
  • The trial was funded by the manufacturer.

The research
behind it.

Ezetimibe consistently lowers LDL and lowers triglycerides slightly. Studies show additive benefit when combined with statins, reducing need for high-dose statin therapy.

Ezetimibe is well tolerated; side effects are rare. Long-term cardiovascular benefit is demonstrated in combination with statins in multiple trials.

Frequently asked questions

Contact support
How do I take Ezetimibe Tablets?

Taken by mouth, once a day. The starting prescription is 1 tablet (10 mg) each time. Your physician sets your own dose and it is printed on your label.

How much arrives in each shipment?

30 tablets at the starting strength, shipped to your door from a licensed US pharmacy.

Why combine ezetimibe with a statin?

Statins reduce cholesterol made by your liver; ezetimibe reduces dietary cholesterol absorbed from your gut. Combined, they hit cholesterol from two angles, often lowering LDL more than either alone. This can reduce the need for very high statin doses, which some people do not tolerate well.

How long does ezetimibe take to work?

Ezetimibe begins reducing LDL within the first week; peak effect is reached around 2 weeks. Full blood lipid response takes 4-6 weeks. Your physician checks labs at 6-8 weeks to confirm efficacy.

Can I take ezetimibe with a statin?

Yes. Ezetimibe and statins work by different mechanisms, so their LDL-lowering effects add together. The combination is often chosen to reduce the need for very high statin doses.

What if my LDL is not low enough?

Your physician may increase the ezetimibe dose, increase the statin dose, or add a third agent (like a PCSK9 inhibitor). Discuss your LDL goal and how to reach it.

Does ezetimibe have long-term side effects?

No. Ezetimibe is well tolerated long-term. Regular liver enzyme monitoring is recommended if combined with a statin. Report any muscle pain to your physician promptly.
