What is fish oil?

(EPA and DHA omega-3 fatty acids)

Fish oil contains EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), omega-3 fatty acids your body cannot make. Your brain, heart, and joints use EPA and DHA to build cell membranes, reduce inflammation, and support signaling. Most modern diets are deficient in omega-3 relative to omega-6, contributing to systemic inflammation. Fish oil supplementation restores balance.

EPA and DHA reduce inflammatory markers

A typical dose is 1,000-2,000 mg combined EPA+DHA daily. Quality matters: molecularly distilled fish oil has lower contaminant levels than crude oil.

How it works,
week by week.

  1. Day 1

Your physician reviews your diet, lifestyle, lab values (including vitamin D level), and goals. Discusses which vitamins fill gaps and how to time them with meals.

  2. Week 1

Start your daily vitamins with breakfast or as directed. Take fat-soluble vitamins (D, K, CoQ10, fish oil) with food to improve absorption.

  3. Weeks 2-4

Most people tolerate vitamins well. Some notice subtle energy shifts or better joint comfort. Keep a symptom log.

  4. Month 2-3

Your physician may check follow-up labs (vitamin D level, if supplementing D) to confirm optimal dosing.

  5. Beyond

Vitamins work best on a consistent daily schedule. Annual labs and check-ins ensure your regimen matches your current needs.

What you may notice,
and when.

  • Better energy

    especially if you were deficient

  • Stronger bones

D and K work synergistically over months

  • Reduced inflammation

fish oil and Q10 support recovery

  • Improved endurance

Q10 and fish oil support exercise capacity

Individual results vary. Benefits reflect what Zepym Health members commonly report to their physician at check-ins; they are not the results of a clinical trial.

what to know before you start.

Important safety information and common side effects

Vitamins D, K, CoQ10, and fish oil are generally well tolerated at recommended doses. D3 at very high doses (over 10,000 IU daily long-term) can raise serum calcium; your physician sets your dose. K2 is safe; no upper limit is established. Q10 is safe; high doses may cause mild GI upset or insomnia. Fish oil can thin blood slightly at high doses; tell your physician if you take anticoagulants or antiplatelet drugs. Fish oil may cause burping or GI upset in some people. Stop and seek care for signs of hypercalcemia (nausea, vomiting, confusion) or unusual bleeding.

The research behind
fish oil.

Omega-3 fatty acids from fish oil were tested in large outcome trials. The largest general-population trial was negative, and the one positive heart trial tested a different, purified product.

  • General populationNo effect on major events

VITAL randomized about 25,000 adults over 50 to 1 g of omega-3 a day or placebo, with vitamin D tested separately, for a median of 5.3 years. Major cardiovascular events and invasive cancer were not significantly reduced.

Manson et al., NEJM 2019 (VITAL, marine n-3 paper)
  • Acting on high-dose EPA25% fewer events, a different product

REDUCE-IT tested icosapent ethyl, a purified EPA product, at 4 g a day in 8,179 statin-treated adults with high triglycerides. The primary composite fell by about a quarter (17.2% vs 22%). This is a different product from fish oil capsules, and the trial was industry-funded.

Bhatt et al., NEJM 2019 (REDUCE-IT)
  • STRENGTHNo benefit, a second null trial

STRENGTH tested a high-dose omega-3 carboxylic acid (EPA plus DHA) against corn oil in high-risk patients and was stopped early for futility. It found no reduction in major cardiovascular events.

Nicholls et al., JAMA 2020 (STRENGTH)

What the studies found

  • Lower triglycerides with high-dose icosapent ethyl in statin-treated adults (REDUCE-IT, NEJM 2019)
  • Fewer major cardiovascular events with icosapent ethyl 4 g a day, a different product (REDUCE-IT, NEJM 2019)
  • An exploratory, not prespecified, lower heart-attack rate with omega-3 in VITAL; it needs confirmation (VITAL, NEJM 2019)

VITAL tested 1 g a day of omega-3 in adults without prior heart disease or cancer. REDUCE-IT tested icosapent ethyl, not ordinary fish oil. The capsules sold here are not the trial product.

What this does not say

  • REDUCE-IT results do not transfer to standard fish oil capsules.
  • VITAL heart-attack findings are exploratory and hypothesis-generating only.
  • REDUCE-IT was funded by the manufacturer of the trial product.

The research
behind it.

Fish oil EPA and DHA reduce triglycerides, support endothelial function, and lower cardiovascular event risk in multiple large trials.

DHA is essential for brain development and maintenance; EPA supports mood regulation. Together, they reduce joint inflammation and support cognitive aging.

Frequently asked questions

Contact support
How do I take Fish Oil Capsules?

Taken by mouth, once a day. The starting prescription is 1 capsule (1,000 mg) each time. Your physician sets your own dose and it is printed on your label.

How much arrives in each shipment?

30 capsules at the starting strength, shipped to your door from a licensed US pharmacy.

Will fish oil make me burp fish?

Quality, molecularly distilled fish oil has minimal fishy aftertaste. Taking it with food, storing it in the freezer, and choosing enteric-coated capsules (which dissolve in the intestine, not the stomach) all reduce aftertaste. Discuss options with your physician.

Can I take all these vitamins together?

Yes. D, K, CoQ10, and fish oil are safe to take together. Take them all with food to improve absorption. Separate any other medications by 2-4 hours from fish oil if you are on blood thinners.

How do I know if I am deficient?

Vitamin D deficiency is easy to test; your physician can check your serum 25-hydroxyvitamin D level. K, Q10, and omega-3 deficiencies are harder to test precisely, but dietary assessment and symptoms guide supplementation.

Are vitamins from food better than supplements?

Ideally, you get vitamins from food. But modern diets often fall short. Supplements fill gaps. A varied diet plus targeted supplements usually works best.

How long until I notice a change?

Energy shifts may appear in weeks; bone and joint benefits take months to become obvious. Be patient and consistent; improvements build gradually.
